Privacy and Legal Notice
The Office of Equal Opportunity and Diversity at the University of California, Irvine is committed to protecting the privacy and accuracy of personally identifiable information to the extent possible, subject to provisions of state and federal law including the state and federal constitutions, FERPA, HIPAA, and IPA. The exceptions to confidentiality are disclosures of child or elder abuse and imminent risk of serious harm to self or the general public.
Information Collection and Use
Categories of personally identifiable information collected by webforms managed or maintained by Office of Equal Opportunity and Diversity may include: date and time our site was visited as well as information you used to fill out forms such as: name, email address, phone number, University affiliation, ucinetid, and the contents of your report submissions. The Office of Equal Opportunity and Diversity uses this information to respond to consultation requests and complaints submitted to our office.
In order to respond to consultation requests and complaints submitted to our office, we may use personal information collected from webforms for the purpose of future communication back to individuals named in online report submissions. In accordance with University policies and procedures, The Office of Equal Opportunity and Diversity may share collected information with other University of California or University of California, Irvine departments including but not limited to, UCI Police Department, Human Resources, Academic Personnel, Office of Academic Integrity & Student Conduct, Office of Whistleblower, and Office of the Ombudsman.
Information collected by The Office of Equal Opportunity and Diversity is subject to RMP-7, “Protection of Administrative Records containing Personally Identifiable Information.” To maintain compliance with federal and state law as well as University policies and procedures, collected information may require an indefinite retention period.
Protection of Information
Access to this information will be limited to the minimum number of people necessary in order to respond to consultation requests and complaints. Technical, physical, and administrative safeguards will be used to prevent unauthorized access to this information. Appropriate investigative and corrective action will be taken in response to a security or privacy incident affecting the information provided on this form.
Disclosure of Collected Information
The Office of Equal Opportunity and Diversity will not disclose or share outside the University of California, without your consent, personally identifiable information, except for certain explicit circumstances in which disclosure may be required by law.
